Pharmacy DirectorsArrowhead Arrowhead, Arizona Dr. Keith Gallus earned his Pharm.D. degree from the University of Minnesota College of Pharmacy in 2003. During pharmacy school, he was involved in the Kappa Psi Pharmaceutical Fraternity and was a recipient of the Eli Lilly Achievement Award. In addition, Keith has a bachelor’s degree in Chemistry from St. John’s University. After pharmacy school, Keith worked for several years at an independent pharmacy in his home state of Minnesota. During this time, he became involved in the American College of Veterinary Pharmacists, and was certified in immunization delivery. In 2007, Keith and his wife relocated to Arizona. As Keith had worked many years in an independent pharmacy, he was looking for a job in Arizona that allowed him to interact more with his patients and keep him in that community pharmacy setting. Since becoming a pharmacist with The Apothecary Shops in October of 2007, Keith has had specialized training in veterinary care and compounding. Keith and his wife live in Glendale and enjoy traveling, movies and golf. Gilbert, Arizona Dr. Kim McCroden earned her Pharm.D. degree from Creighton University School of Pharmacy in 2006. During pharmacy school she was actively involved in student government, the Kappa Psi Pharmaceutical Fraternity, the American Society of Health System Pharmacists, and the National Community Pharmacists Association. After pharmacy school, Kim relocated to Arizona from her home state of Nebraska, and joined the Apothecary Shop staff. Having worked many years in a retail pharmacy chain Kim was looking for a job that allowed her to focus more on patient care, and have the ability to specialize in different areas. Since becoming a pharmacist with the Apothecary Shops, Kim has become certified in immunization delivery, and has had specialized training in diabetes management, veterinary care, and compounding. In 2004, he earned his doctoral degree from Midwestern University College of Pharmacy in Glendale, Arizona, graduating summa cum laude. Dan specializes in Infectious Diseases, transplant treatments and general pharmaceutical compounding. Dan is a graduate of the University of California – San Diego, where he studied biology. He and his wife Jennifer live in Phoenix and enjoy playing golf, mountain biking and reading. Phoenix, Arizona (17th Street Location) Jim Rehovsky earned his pharmacy degree from North Dakota State University in 1986. He joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio, in May 1999. Prior to joining the company, he was the Director of Pharmacy at Lahr Pharmacy in Phoenix. Jim’s area of expertise includes ophthalmology and fertility. He speaks regularly on ophthalmology including at the annual meeting of the American Society of Ophthalmic Registered Nurses (ASORN). He is an active member of the Arizona Pharmacy Association (APhA). Jim, his wife and three children live in Phoenix. His hobbies include golf, snowboarding, hunting and motorcycling. Jim can be reached at (602) 252-6120 or by e-mail at jrehovsky@theapothecaryshop.com Scottsdale, Arizona (91st Location) Dr. Courtney Yee earned her Doctor of Pharmacy degree from the University of Arizona College of Pharmacy in 2002 and subsequently completed her specialty residency in ambulatory care at the Carl T. Hayden VA Hospital in Phoenix. As a part of her residency, Courtney managed individual patient treatment therapy and gave monthly lectures to patients on topics such as diabetes, congestive heart failure and pulmonary disease. She also has presented to medical residents, nurses and pharmacist groups on oncology and ambulatory care topics. Courtney joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio, in 2004 after moving from San Diego where she was an ambulatory care clinical pharmacist with the San Diego VA Healthcare System. Courtney’s areas of expertise include fertility services, bioequivalent hormone replacement therapy, anticoagulation therapy, oncology, pain management, ambulatory care treatment and HIV/hepatitis treatment. She also serves as a vitamin supplementation consultant for The Apothecary Shops’ patients. A licensed pharmacist in California and Arizona, Courtney is also certified to administer vaccinations through The Apothecary Shops. Courtney can be reached at (480) 451-3771 or by e-mail at cyee@theapothecaryshop.com Linda Cappellini, R.Ph., FACA, FIACP Linda Cappellini received her pharmacy degree from Temple University School of Pharmacy in Philadelphia where she was a member of The Rho Chi National Honor Society. In 1996, Linda joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio. Her dedication to the profession is evident with several certifications in compounding, including women’s health, aseptic and sterile products, and pain management. She currently supervises the Compounding Quality Assurance program for the entire Apothecary Shop organization, which is responsible for more than 6,000 compounded prescriptions per month. Linda is an Adjunct Clinical Preceptor for University of Arizona College of Pharmacy in Tucson, Ariz., and Midwestern University College of Pharmacy in Glendale, Ariz., and in 2002 was honored as Preceptor of the Year. She is a full fellow in the American College of Apothecaries (ACA) and is a member of the International Academy of Compounding Pharmacists (IACP), the American Pharmacists Association (APhA) and the North American Menopause Society (NAMS). She also is NIPCO certified in Osteoporosis Care. During the last five years, Linda has focused her expertise on Women’s Health Care. She has completed numerous courses and has received various certifications on the subject. Linda is the only pharmacist in Arizona who has achieved both Certified Menopause Educator and Certified Menopause Practitioner degrees. She comes from a family of three generations of pharmacists and has 27 years of pharmacy experience. She has completed extensive course work in the area of bio-identical hormone replacement therapy, compounding and counseling. After graduation, Susan evaluated the many opportunities available in the pharmacy field. She has experience working in hospitals, retail, specialty, HMO and compounding pharmacies, and also has held supervisory positions throughout her career. In 2003, Susan joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio. During the last 10 years of her career, Susan has focused her pharmacy experience on compounding, women’s health, oncology, fertility and veterinary medicine. As a breast cancer survivor, Susan is able to share, comfort and touch the lives of her oncology patients by drawing on her personal experiences and firsthand knowledge. He has also completed graduate coursework toward his master of science degree in pharmaceutical economics, outcomes research and health policy at The University of Arizona College of Pharmacy in Tucson. Following pharmacy internships with Health Alliance Plan in Detroit and PCS Health Systems in Scottsdale, Ariz., Jamie worked as a staff pharmacist with Osco Drug Stores in Tucson. In the process, he came to recognize the difficulty experienced by many people--particularly seniors--in paying for medications and staying healthy. The small-town atmosphere and relationships of a pharmacy offered him the ideal venue to make a difference in peoples’ lives. As a student, he twice received national recognition by the American Society of Health-System Pharmacists (ASHP) and earned a Rho Chi certificate of merit. Following a pharmacy internship at Tucson Medical Center, he completed his pharmacy practice residency at the Southern Arizona VA Medical Center in Tucson, where he served as a clinical weekend pharmacist and IV room pharmacist. In November 2001, Eric left his position as a staff pharmacist at University Medical Center in Tucson to join The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio. His areas of expertise include oncology, pain management and infectious disease. In addition to being a member of the adjunct faculty at University Medical Center, where he teaches fourth-year pharmacy students, Eric is a member of the ASHP and American Pharmacists Association. His research experience has included studying the management of anticoagulated patients and trends in antidepressants, and he has presented to professional audiences on a number of health topics. Eric is also active in community education, and has frequently presented to the Arizona Deaf and Hard of Hearing Society. As a student, she served as Kappa Epsilon Pharmacy Fraternity President, was a member of The Rho Chi National Honor Society and received The American Pharmacists Association-Academy of Student Pharmacists (APhA-ASP) Professionalism Award. She currently serves as an extern preceptor (teaching students in their final year of training) at the University of Arizona. After graduation, Crystal completed her pharmacy practice residency at the Seattle, Washington VA Medical Center where she served as a clinical pharmacist and IV room pharmacist. In 2000, Crystal moved back to Tucson where she joined the pharmacy staff at the Tucson VA Medical Center. There she served as a pharmacist in the ambulatory care/telephone link care unit. He also is certified in Acute Homeotherapeutics and is working toward becoming a Bach Flower Therapist. Ram was drawn to the pharmacy profession because of his desire to continue the legacy left by his grandfather who owned a pharmacy in Poland. After college, Ram worked for a chain pharmacy and home infusion companies on the east coast. He returned to his native country of Israel, where he was involved in the start-up of several health care companies, and spent five years as a pharmacy manager for a leading chain pharmacy. Ram is licensed in Nevada, New Jersey and Oregon. In October 2007, Ram joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio. Prior to joining the company, he was a pharmacist for seven years at a leading chain pharmacy. Ram resides in Las Vegas with his wife Gabi and their two children. He has more than 17 years of pharmacy experience. In September 2006, David joined The Apothecary Shops, a family of independent, community pharmacies located throughout Arizona, Nevada and Ohio, and opened the company’s second out-of-state pharmacy. Prior to joining the company, he was a pharmacist at New Albany Surgical Hospital. David’s areas of expertise include ophthalmology, women’s health and diabetes. Pharmacist Mike Berntsen earned his pharmacy degree from the University of Utah in 1971. However, his career in pharmacy started long before that when he worked in his father’s pharmacy beginning at age 10. In February 2008, Mike joined The Apothecary Shops as pharmacy director of the company’s second out-of-state pharmacy. Mike brings a tremendous amount of pharmacy, compounding and managerial experience to The Apothecary Shops. He owned and operated a pharmacy in Provo, Utah for 32 years. After selling the pharmacy in 2003, he worked in retail pharmacy for three years, and also acted as the supervising pharmacist for Project Reality Methadone Substance Abuse Rehabilitation Clinic for more than 10 years. During his career, Mike was also a three-time President of the Utah County Pharmaceutical Association. Mike has seven children and eight grandchildren.
